As part of our updated policy and in preparation for our online booking system, we will kindly be asking that you fill out our Client Intake Form. This will include an agreement and liability waiver while the Payment Form will give us authorization to hold a credit card on file. You will be required to create a Client Profile with a Vagaro account which can be done via the link or by downloading the Vagaro app in the Apple App Store. You may also download the Vagaro app in the Google App Store.
